Your starting point for home care

The support we provide

Care shaped around
the person’s needs.

You do not need to identify the right service before contacting us. Tell the team about the support needed, where the person lives and the care already in place. We help organize an appropriate assessment and explain the next steps.

Our office is the coordination point for home-care services. It handles enquiries, scheduling and care arrangements; medical treatment and clinical home assessments are carried out by the relevant professionals in the appropriate setting.

Initial enquiry and guidance

A conversation about the care needed, location, existing support and the most appropriate assessment route.

Assessment arrangements

Coordination of the relevant nursing, rehabilitation or personal-care assessment, based on the needs described.

Individual care planning

An agreed plan that identifies goals, services, responsibilities, visit frequency and review points after assessment.

Visit scheduling

Coordination of visit arrangements and family contacts, with clear communication about timing and responsibilities.

Communication with existing providers

Coordination of relevant referrals and care information through agreed confidential channels and with appropriate patient involvement.

Review and changes in care

How care is arranged

  1. Step 1

    Contact the coordination team

Explain the support needed and the person’s location. We discuss relevant services and assessment arrangements.

  2. Step 2

    Complete the appropriate assessment

The relevant professional reviews needs and works with you to agree goals, services, visit arrangements and costs.

  3. Step 3

    Stay connected as care continues

The team coordinates the agreed support and helps arrange review when circumstances or care needs change.

Preparing for an assessment

  • Prepare a short description of the support needed, the person’s location and a contact number.
  • Identify existing providers or recent discharge information that may be relevant to assessment.
  • Have questions ready about care options, visit schedules, family involvement and costs.

Keep medical records and detailed health information out of general email enquiries. If an assessment is arranged, the team explains how to share information appropriately.
