Connecting professionals, families and home care

Referrals and partnerships

Refer a patient for suitable home care in Accra or discuss a programme for your organisation. Glorious Health Care Services GH works with patients and families to coordinate support after discharge and in daily life.

Our office in Dome coordinates referral enquiries and home visits. Confirm service acceptance, consent and handover arrangements before sending patient information.

Hospitals, clinics and professionals

Help connect discharge plans with care at home.

Leaving hospital can bring new care routines, mobility challenges and questions for a family. GHCS provides a point of contact for home nursing, post-discharge follow-up, chronic-condition support, rehabilitation and personal care. Assessment establishes which support is suitable for the individual patient.

We review referrals against the patient's needs, clinical stability, service scope, location and staffing. Confirm GHCS acceptance before relying on an onward care arrangement. A referral enquiry does not itself establish that visits are booked.

  1. Discuss the referral

Start with a general professional enquiry. Agree the service requirements, contact responsibilities and visit arrangements before sending patient information.

  2. Confirm consent and a secure handover

After service acceptance, use an agreed secure channel with appropriate patient consent or lawful authority. Relevant discharge instructions, medicines, care goals and risks inform assessment and continuity of care.

  3. Assess and agree a care plan

A suitable professional assesses needs and agrees the tasks, schedule, payment responsibilities and escalation arrangements with the patient.

  4. Coordinate follow-up

Agree the relevant progress updates and contacts, with the patient's consent or lawful authority. Review changing needs and refer back when they exceed the agreed home-care scope.

Patients and families can start an enquiry too.

A professional referral is not required to ask for general information. Whether a service needs a clinician's referral or order depends on the care involved. Explore the guide to care for preparation and family involvement.

Shared ambitions

Build around a defined community need.

Employers

Discuss health education, appropriate screening and support for employees balancing work with family caregiving. Scope, confidentiality and funding need agreement before a programme begins.

Community organisations and NGOs

Discuss older-person support, disability inclusion, caregiver education or chronic-condition programmes shaped around local priorities and appropriate clinical oversight.

Metropolitan, municipal and district assemblies

Discuss community outreach and programme opportunities with MMDAs and other public bodies. Collaboration requires the relevant permissions and written responsibilities.

Insurers and programme funders

Discuss agreements specifying eligible services, patient eligibility, payment terms and reporting. GHCS currently provides private-pay care and does not accept NHIS. We intend to accept NHIS in the future. Any partnership funding arrangement needs written agreement before care begins.

A programme should identify who it serves, the services involved, referral and escalation routes, staffing, funding and how results will be assessed. Measures could include completed visits, care-plan progress, patient feedback and safe onward referral. Agree the measures and reporting responsibilities at the outset; patient outcomes vary and should be assessed against the individual's care goals.

Start with the purpose of the partnership.

Contact us with your organisation, role, contact details, the general location and the type of referral or programme you want to discuss. Outline the population or care gap involved without identifying patients.

There is no public patient-record upload on this website. Do not send medical records, prescriptions, identity documents or patient lists by business email. Patient-specific handover requires acceptance and an agreed secure process first.
