What your care assessment explores
An assessment considers the person’s goals, current treatment, daily activities, mobility, family support and home environment. Nursing, medical or therapy input is arranged according to the needs identified. It helps us understand what can be supported safely at home and when another provider’s input is needed.
Your care plan sets out the agreed support, responsible professionals, visit arrangements, costs and review points. We discuss referrals or escalation where needed. The patient’s consent and preferences guide family involvement, and the team confirms the assessment appointment and care start date with you.
Have information ready for an agreed assessment
Think about the support needed, the person’s usual routine, access to the home and any questions for the team. Relevant discharge instructions and medication information may be needed later. The team explains how to provide relevant information through the assessment process; please do not attach them to a general email.
Confirm costs and availability first
Care is currently private-pay. GHCS does not currently accept NHIS, although we plan to do so in the future. Ask about assessment fees, visit charges, travel and any materials or equipment so you understand the total arrangements before agreeing to care.
